FORUM HOUSE RABBIT Q & A Bunny Head Tilt – Ear Infection or EC?

        Hi everyone,

        My female holland lop Bubba was spayed on Friday and the vet let me know the surgery went well. When I brought her home that evening I put her in a small pen in my room to limit her activity during recovery. About 5 minutes after putting her in there she began to fall over to one side and could not get back up. She seemed to seriously freak herself out. We managed to pick her up but she was somewhat limp and still leaning to one side. Once we put her down she seemed fine and upright again but we decided to keep her in her carrier for a bit to try and limit her rolling over. She loves to flop over onto her side in order to lay down and we just figured since she was sore she was unable to so we weren’t too concerned yet. She made it through the night just fine and in the morning we tried putting her in the pen again but the same thing happened after a few minutes. We still figured maybe she just wasn’t ready to move around much and since they had tried using her arm for an IV it was quite raw so we thought that could be why as well. Anyway that evening the rolling over was happening more frequently and I noticed her head was tilted so I took her to the ER where they checked her vitals which were all good cause she has still been eating and drinking normally and they thought she might have an ear infection. Unfortunately the vet there wasn’t very experienced with rabbits and they had no medication for rabbits that they could give us. So we took Bubba home and watched her all night. We noticed she fell over once or twice but usually managed to get herself back up. This morning I took her to a different ER that had a vet who could help rabbits and she said it is also likely an ear infection but without doing tests she couldn’t rule out EC or a tumour. She gave me antibiotics and advised me to take Bubba to her regular vet tomorrow so she can run more tests. I’ve given Bubba some of her antibiotics and will give more before bed. The rolling over seems to be under control and her head isn’t constantly tilted but it does tilt occasionally. Also it looks like her eyeballs are moving vertically and she has some gunk around her eyes. 

        I have done some reading about the difference between EC and ear infections and I am just trying to figure out what is more likely in her case. To me an infection would make sense because her coincidentally developing EC the same day she is spayed just seems to strange. Although an ear infection is strange too honestly. So if anyone has experience with an inner ear infection or EC and could let me know if any of these symptoms are familiar I would greatly appreciate the help!

          My Bombur has EC and it correlating with the spay isn’t unlikely. Bunnies with EC won’t show symptoms until their immune systems are under stress, like during a major surgery such as a spay. So it could very likely be EC. EC can come in many shapes and sizes, so that’s also something to be aware of. Some bunnies fall over and have head tilt and others only have minor symptoms like incontinence and hind leg weakness. EC can also test as a false negative, so there’s truly no way of knowing for certain if it could be EC unless a positive came back. Meaning she could have EC and still test negative. If she has EC a standard treatment is a 28 day course of Panacur or Oxibendazole. Bombur has been on 3 runs lf Oxibendazole. So if she has it, it’s a very manageable disease and she can still live a long and happy life.

            Thank you so much for your response! We took her to the animal emergency and were given antibiotics for an ear infection – to be taken twice a day for one week. Since starting the antibiotics she has gotten SO much better No more falling over or head tilt. I am thinking it must be what was causing it because if it was in fact EC the antibiotics likely wouldn’t make a difference? The vet explained ear infections can come up in similar situations as EC such as when they are under stress which would make sense with the spay. 

            Also, how is your Bombur doing? Has the medication treated the head tilt? I’ve heard that in some cases it will eliminate the parasite but the bunny can still be left with a head tilt but they are still so happy and full of life! I think it makes them unique

              That is very strange. I had a bunny start to show EC symptoms after a 3 day stasis episode. But never heard of it from something like a spay. I think you are on the right track, for sure, if she has already improved! Make sure to keep a close eye on her, even when she is done her antibiotics, because ear infections can be stubborn and often need several rounds of treatment.

                Very strange indeed…there were many complications I was prepared for but this was not one of them! I will definitely keep an eye out once the antibiotics are done in case it seems to be coming back. Thank you for your reply
