He has these episodes where he doesn’t eat or drink for a few hours. We went to the vet twice before and after pain killer and a bit of critical care, etc he gets back to normal after a couple hours.

But this is something I haven’t seen yet. I already gave him pain med and a tiny bit of critical care/water (because he was trying REALLY hard to get away this time which indicates that he isn’t THAT sick, hopefully) so  now we are just observing him and since the pain med started working already he should have started eating or drinking but he just goes to the hay bowl, sniffs it and goes away and same with water, he was lying beside the water bowl and licks it once but doesnt drink it.

We are probably going to take him in to get injected with water, but has anyone else ever seen this and what could it mean?

Has the vet checked his teeth? Without knowing anything else, it sounds like he might have a dental problem. That would also explain why he REALLY didn’t want you syringe feeding him (his mouth might be hurting him).

Deep molars are impossible to see without anesthesia, so if he has recurring episodes with no other explanation it sounds like a dental exam is the next step.
